What is PT 33?

PT 33 is the official controlled-herb prescription form. A licensed practitioner issues it to a patient, stating the daily dosage, number of days and total quantity. Per the notes on the form, one prescription covers at most 30 days, and the dispensing premises must keep it for inspection for 1 year from the dispensing date

How to fill in each field

  1. Date — the prescribing date
  2. Prescriber — name, then tick one practitioner type
  3. Licence/certificate number and the prescriber's address
  4. Patient — name, age, nationality and ID card or passport number
  5. Conditions — as diagnosed
  6. Daily dosage (g) and number of days, then the total quantity
  7. Signatures — the patient and the prescriber sign on paper

Common mistakes

  • Practitioner type not ticked, or licence number missing
  • More than 30 days on one prescription
